You can get the eye test done at any hospital for a nominal fee, just bring two passport photos with you.
Then bring all the documents plus 360 AED to any driving license centre and they'll sort it out for you. if you're in Dubai I've heard you can also have it done in the basement area of the Marina mall, but not a hundred percent sure. Still, might be worth checking it out.
